Eligibility: SLMTA’s success was made possible by efforts from thousands of people all over the world. Many contributed with what they had, doing the best they could in whatever position they were in. This award is for anyone who has contributed to the success of SLMTA, and particularly for those “unsung” heroes. No impact is too small. We will accept all nominations provided the nominations are well written and the impact (how his/her work made a difference) can be validated.
Nominators: A nominator may be the nominee’s supervisor, subordinate, co-worker, partner, client, or customer, who has first-hand experience of the nominee’s work.
Nominations: Fill out the submission form providing all required contact information for you and your nominee. Submission of the nominee’s photograph is also required. We will also accept video (less than 5 minutes long) as a form of submission.
- Nominations must be submitted on behalf of another person. Self-nominations will not be accepted.
- Nominations must be submitted in English only.
- Nominations may only be submitted in the name of a single person.
- If a single person’s contribution cannot be separated from another individual, then on this rare occasion we will accept group nominations.
- Nominations of an entire organizational unit (such as a laboratory or a department within a laboratory) will not be accepted.
False or deceptive nominations or acts may render a nominee ineligible. We reserve the right to edit the nomination as necessary for publication on the SLMTA website. By submitting a nomination you and your nominee agree to grant the SLMTA website the right, without obligation, unless prohibited by law, to use the contents of your nomination, your name, voice, picture and likeness, without compensation, for the purpose of publicizing all matters related to the SLMTA Hero Program and/or the SLMTA website in any medium, throughout the world in perpetuity.
Nominees may be contacted to provide additional information and have their information verified. The SLMTA Hero Program reserves the right to request and require additional information from nominees in order to have their nomination considered.
This award is solely for recognition purpose. No monetary prize will be awarded to the SLMTA heroes.
Acceptance Requirements: A nomination will be accepted if it provides a detailed, thoughtful entry describing accomplishments and impact of the nominee’s work (how his/her work made a difference).
